Privacy Policy

What the appointment card collects
When you submit the appointment card on the contact page, the office receives the day you circled, whether you asked for a morning or an afternoon, the kind of visit selected, your name, your phone number, whether you marked yourself new or returning, and anything you typed in the free-text box.
That box is deliberately small. Please keep it to a sentence about what hurts. Detailed history, medication lists and anything you would consider sensitive belong in the room, not in a web form.
Why it is used
Only to call you back and put a time in the book. The information is not sold, rented, traded, or used to build an advertising profile, and it is not shared with a marketing platform.
How long it is kept
Appointment requests that do not turn into a visit are discarded within ninety days. If you do become a patient, the relevant details are transferred into your clinical record, which is retained for the period New York State requires of a chiropractic practice.
Cookies and measurement
This site sets no advertising cookies and runs no third-party tracking pixels. Fonts are requested from Google Fonts, which means your browser contacts Google's servers to fetch them; that request carries your IP address in the ordinary way any web request does. Standard server logs are kept briefly for security and troubleshooting.
Who else can see it
- The office staff who answer the phone and manage the book.
- The hosting provider that runs this website, as an unavoidable technical matter.
- Anyone we are legally required to disclose to, for example under a valid court order.
Insurance carriers, no-fault adjusters and attorneys receive information only from your clinical record and only with the authorisation you sign in the office — never from this website.
Your choices
You can ask what the office holds from a web submission, ask for it to be corrected, or ask for it to be deleted. Call or email and it will be handled; there is no form to fill in for that.
Children
The appointment card is meant to be completed by an adult. When a visit is for a child, the parent or guardian submits the card and attends the appointment.
Security, honestly stated
The site is served over HTTPS and submissions are handled by the office rather than a third-party marketing tool. No website can promise perfect security, so treat the free-text box as a note, not as a confidential channel.
Changes to this page
If the way the card works changes, this page is updated and the date at the top changes with it. Material changes will also be mentioned at the desk.
